1. Letter of submission
2. Registration Certificate of the firm.
3. Up to date Trade License
4. VAT Registration Certificate.
5. Tax Identification Number (TIN).
6. Brochure of the consulting firm summarizing their area of expertise.
7. Overall diversified experience of the firm.
8. Bank statement along with Bank Solvency Certificate.
9. Availability of appropriate professionals (List of relevant professional staff, their qualification & area of expertise to be furnished).
10. The Firm should have minimum 20 (Twenty) years of experience in the field of architectural, engineering and project management consultancy services.
11. The firm should have completed at least 02 (Two) contracts within last 10 years in specific experience of design & construction supervision of Hospital/Medical College/University.
12. The firm should have completed Hospital /Medical College/University design and construction supervision with contract price at least 350 (three hundred fifty) lakh in a single contract within last 10 (ten) years.
13. The firm should have completed design & supervision for a minimum 10 (ten) storied hospital building.
14. The firm should have completed design and construction supervision for a minimum 500 Bed hospital. |

Duration of the Service It Should be harmonic and compliance with the stipulated time for the project to be completed.
Other details i) Consultant will be selected in accordance with PPA 2006 and PPR 2008 and public Procurement Procedures issued by GOB with its updated amendment.
