All Bidders must submit Tender through “Tender Booklet” prepared by procuring entity and abide by the terms and conditions stated in the “Tender Booklet”.
The Bidder must enclose the following documents with the Tender and also have to show the treasury challan with Online Chalan Verification copy when purchasing the tender booklet.
1 The bidder must be a Bangladeshi Citizen and must submit attested photocopy of National Identity Card (NID).
2 Attested 02 (Two) Passport size photos of the bidder/ representative.
3 Attested Valid Trade License.
4 Attested Up-to-date13 digit Business Identification Number (BIN) Certificate.
5 Attested 12 digit Electronic Tax Identification Number (E-TIN) registration certificate.
6 Income Tax Clearance Certificate & Acknowledgement Receipt of Income Tax Return of current fiscal Year.
7 Bank Statement (Original Computer Sheet) From 01-03-2022 to 28-02-2023. Bidders bidding for different groups should attach the original copy of the bank statement with one group and attested photocopy of the same with other groups.
8 Financial Solvency certificate stating amount from any reputed Bank (The statement should be issued after 01-03-2023). Certificate Stating “Sound & Solvent” is not acceptable and bids submitting that type of certificate will be rejected.
9 Status of the Bidder as an Importer/ Contractor/ Supplier/ Distributor in firm’s Letter head Pad.
10 The Bidder should be experienced in supplying MSR Goods worth at least total group: A & B BDT 30,00,000.00 (Thirty Lac only) and group: C, D, E & F BDT 15,00,000.00 (Fifteen Lac only) to any 100 bedded Upazila Health Complex/ Office of Civil Surgeon/ District Hospital/ General Hospital/ Medical College Hospital/ Government Specialized Hospital in any 02 (Two) financial year out of last 05 (Five) financial years completed contractors can submit tenders. Experience certificate and respective work order must be submitted.
